Facility Usage Form - Beacon Hill Church

PLEASE READ

BHC facility is only available for usage that directly and primarily promotes BHC ministry (NOT personal use). As good stewards of the church, please do not submit requests that may be more personal in nature but disguised as ministry. For example, refrain from submitting a usage request if you can do the event outside of the church (e.g. a home) but you want to use church facility to accommodate people not directly connected to the ministry.

Only ministry leaders may request church facility usage. If not a ministry leader, please ask your ministry leader to submit this form.

For wedding and memorial services, facility usage is only available for BHC worshippers. The officiant of the event must be approved by the pastoral staff. All exceptions will be reviewed on a case by case basis.

Step 3: you will get an email stating it has been reserved or denied within 48 workday hours (M-F)

What is the name of this event? Village Christmas Party, small church baby shower, youth graduation party, etc?

Usage Purpose *

Explain the primary ministry reason and the purpose of the usage. For example, do not write outreach as the primary when in fact outreach is just incidental.

Usage Description *

Describe how the facility or property will be used. (e.g. kitchen to prepare food for Saturday Youth gatherings)

How many are you expecting at this event? *

Provide a rough breakdown of who is coming. *

For example, 6 small church members, 1 VIPs, and 5 other BHC/CBC members, and 25 friends and family.
